FACIS Level 3 (Fraud and Abuse Control Information System) is designed to assist personnel in precluding employment of an excluded individual or entity where reimbursement is sought from any federal health care program. This Level 3 FACIS® search also contains information from all 50 states on individuals who have been the subject of state licensing board sanctions.
